Application for Online College Admission for

New Enrollees and Transferees

(2nd Semester, A.Y. 2021-2022)

2. Complete all of the required information.
   Reminder:
   • Review the information you have provided for typographical errors
   • Provide a valid contact information
3. Upload 1×1 picture with white background, scanned copies of your report card (Form 138) and good moral certificate.
   Note:
   • If your report card isn’t available yet, report card with grades from the 3rd period will do.
   • The original hard copy of requirements will be submitted upon request of the Admission and Testing Office, the University Registrar, and/or College
4. After your application has been submitted, the Admission and Testing Office will send you a confirmation email with your admission number.
   Note:
   • Take note of your admission number. It will be required during the enrollment process.
5. Within 10 days, another e-mail will be sent to you by the Admission and Testing Office regarding the approval of your application. Fill out the Routine Interview Form while you wait for the e-mail: https://forms.gle/zTegNMMa3t9DXdoC7
6. Track the status of your application through this link: https://inq.dlspsanpablo.com
7. Proceed to enrollment steps once you’ve been approved. Enrollment schedules will be announced.
