APPLICATION REQUIREMENTS

First-Year College (New Enrollee)

  • Form 138 – Report Card (Must have at least grades from the first third grading period of Grade 12)
  • Form 137
  • Certificate of Good Moral Character
  • PSA Birth Certificate
  • One (1) piece 1×1 Colored Picture with White Background

Transferee

  • Transcript of Record (TOR) or Copy of Grades for evaluation purposes
  • Honorable Dismissal
  • Certificate of Good Moral Character
  • PSA Birth Certificate
  • One (1) piece 1×1 with White Background

Note:

  • TOR of a transferee student will be subjected for evaluation by the Registrar or College of his/her program of interest.

Degree Holders and Teacher Certificate Program (TCP)

  • Transcript of Record (TOR)
  • PSA Birth Certificate
  • One (1) piece 1×1 Colored Picture with White Background

Alternative Learning System (ALS)

  • A & E Results
  • ALS Diploma
  • Certificate of Passing A & E Examination (Division Office)
  • PSA Birth Certificate
  • One (1) piece 1×1 Colored Picture with White Background

Returning Students (A.Y. 2020-2021 and previous academic years)

  • Accomplished Clearance Form
  • Evaluation Form from the Registrar/College

APPLICATION PROCESS

1. Access the Online Admission link.

2. Complete all the required information.

Reminders:

  • Review the information you have provided for typographical errors
  • Provide a valid contact information
  • Type “NA” if the question does not apply to you

3. Upload the scanned copy of your admission requirements.

Reminders:

  • Scan your credentials using a scanner-equipped printer or a mobile application such as CamScanner
  • Files must be in JPG or JPEG format and cannot exceed 5MB in size
  • Ensure that the scanned documents are readable and uploaded in the proper orientation

Note:

  • For SHS: If a complete Form 138 or Report Card is not available, a Report Card with grades from the 3rd period will do

4. After your application has been submitted, the Admission and Testing Services Office will send you a confirmation email with your admission number. Take note of your admission number. It is required during the enrollment process.

  • You can track the status of your admission application using our Online Tracker.

5. Following the evaluation of your application, the Admission and Testing Services Office will send you another e-mail or text message regarding your entrance examination schedule within 7 days.

6. Take the entrance examination at your scheduled time at the Admission and Testing Services Office. Late applicants will have their entrance examination rescheduled. You may only take the entrance exam once; retakes are not permitted.

Reminder:

  • Bring a black pen.
  • Bring one (1) long brown envelope and a long white folder.

Note:

  • Your application will be denied if your credentials and/or the results of your entrance exam do not meet the minimum grade requirement for your desired program. The Admission and Testing Office will suggest program(s) to which you may re-apply (no more entrance exam).

7. Fill out the Student Profiling Survey.

8. Upon passing the entrance examination and submitting of complete requirements, your admission will be approved. Applicants who passed the entrance examination, but submitted incomplete requirements will receive an email requesting additional documents.

9. Once approved, Proceed to the enrollment steps.

Reminder:

  • Once enrollment opens, only applicants whose applications have been approved can enroll at the university.
  • The original hard copy of requirements will be submitted upon request of the Admission and Testing Services Office, the University Registrar, and/or College. Applicants who have already submitted an online application with incomplete requirements can submit their additional requirements using this link. Simply enter your Admission Number (AN) and your last name.
